Registration Information for the 2024-2025 School Year
The 2024-2025 Registration Packet can be found in its entirety below or you can print out individual pages of the packet, as needed.)
How to register for preschool:
*Please print the entire registration packet (10 pages in all) and complete each form.
*Please submit cash/check made payable to St. John's Preschool in the amount of $80.00.
*Submit a copy of your child's birth certificate.
*Submit a copy of your child's latest physical and immunization record, including a screening/test for TB and Lead (a current physical is due by the first day of school in September for new students).
